Community Health Worker – Ryan White HIV Program (Full-time, 40 hours per week)

Under the supervision of the Community HealthNet, Inc.’s (CHN) Director of HIV/AIDS Program the HIV/AIDS Community Health Worker (CHW) is responsible for providing outreach, peer support, and education services within the scope of given projects to meet the program requirements of certain grants awarded to Community HealthNet, Inc. The position requires strong interpersonal skills to make connections and build relationships with the target patient/client base. The Community Health Worker works collaboratively with other departments at CHN, as well as other community-based agencies with the purpose of making the critical linkages to health information and services for community members.

As an advocate for community health the CHW also works to educate the public on the services available at CHN and eligibility requirements. The CHW in conjunction with other departments’ clinical staff is involved in direct recruitment of participants for their specific projects. The Community Health Worker monitors their progress through the programs and reports outcomes to CHN’s Director of HIV/AIDS Program on program outcomes.

Community Health Workers use their unique position as trained community members to provide trust-based information and education, help clients (patients) cope with and overcome fear, walk clients through the often complex healthcare system, and help them strategize to manage the logistics of caring for oneself in the context of a complicated life.


  1. Identify individuals who know they are HIV-positive but are not in HIV medical care by receiving referrals, utilizing organizational information about missed appointments, and conducting outreach at various community organizations and venues.
  2. Work to build the trust and confidence of clients.
  3. Participates in, coordinates, and/or attends community events such as health fairs and community forums, as a representative of CHN.
  4. Conducts community presentations and other necessary outreach activities in CHN clinics’ service areas in Lake County Indiana.
  5. Provide accurate information to clients about living with HIV, preventing further transmission, the benefits and challenges of HIV care and treatment, and available services and how to access them.
  6. Together with case managers and clinical staff, support clients throughout the early part of their care so they participate fully, become educated, and can advocate for themselves.
  7. Help clients to develop confidence about their participation in HIV treatment.
  8. Work with case managers to assist clients in identifying services to address other aspects of their life that affect their health, such as mental health and substance abuse treatment, domestic violence services, housing programs, and education and employment services.
  9. Submit clear and accurate data collection forms for all work performed.
  10. Comply with all program protocols, including safeguarding and maintaining strict confidentiality of data and regarding client information.
  11. Works closely with the Case Manager, Care Coordinator, behavioral health staff, and all other CHN Medical and Behavioral Health departments to assist in the integration of services for CHN’s patients enrolled in the Ryan White HIV/AIDS Program (RWHAP).
  12. Provides peer support and navigation of HIV Medical and Behavioral Health services to ensure HIV positive patients are obtaining medical and behavioral health care, and consistently making and keeping their appointments.
  13. Provide personalized assistance to clients to help them enroll or re-engage in medical care.
  14. Participates in mandatory department meetings, trainings, forums, and other educational activities related to specific programs, to build and maintain current knowledge base of awareness and prevention techniques in the specific area of health issues addressed by each project/grant.
  15. Performs home visits as needed and conducts a risk assessment check list as designed and developed by the HIV/AIDS Program Director, to determine risk factors for HIV patients. Will report and review risk assessment check list with the Director of HIV/AIDS Program for development of a “future contact plan” for HIV/AIDS patients.
  16. Provide weekly, monthly, and quarterly reports to the Director of HIV/AIDS Program regarding job functions connected with duties performed in the RWHAP.
  17. Follows up with HIV patients that are referred to internal and external services to help ensure patient referral follow through. Assists patients with connecting to referral sources, if necessary.
  18. Maintains documentation of the program’s activities and assists in tracking relevant data.
  19. Maintains inventory of health education/outreach materials necessary to conduct outreach efforts and to supply providers and other CHN staff as needed.
  20. Attends and participates in all appropriate CHN staff meetings, work groups, etc., as directed.
  21. Performs all duties and services in full compliance with CHN’s Service Excellence Standards.
  22. Performs all duties in support of successful EHR/EPM implementation.
  23. Comply with and successfully complete all required and applicable federal and/or state program/grant training and participate in ongoing conference calls, webinars, and other professional development opportunities.
  24. Maintain knowledge about trends and research applicable to Community Health Workers.
  25. Complete other work-related forms in a timely manner and check company e-mails every morning and afternoon before the end of the workday.
  26. Obtain a Certified Community Health Worker’s (CCHW) certificate within 90 days after the probationary period.
  27. Complete thirty (30) Continuing Educational Points (CEPs) prior to the expiration date on your CCHW certificate.
  28. Performs other duties or tasks as directed by CHN’s HIV/AIDS Program Director.



  • High school diploma or GED Required
  • Associate or bachelor’s degree in health, Social Services, or Marketing field preferable; or two years related outreach experience and/or training, or equivalent combination of education and experience.

  • Bilingual in Spanish preferred
  • Exhibit excellent customer service skills.
  • Ability to use a variety of office equipment and computer programs including Excel, and Microsoft Word software.
  • Maintain confidentially and complies with HIPAA regulations.
  • Dress professionally and appropriately
  • Available for occasional weekend and evening hour outreach activities
  • Ability to pass a pre-employment drug screen and background test.
  • Dependable motorized transportation, valid driver’s license, and proof of auto insurance

  • Ability to work on disease issues that involve diverse social issues such as incarceration, substance abuse, homelessness, migrant workers, sexual minorities, and commercial sex workers.

  • Occasionally lift and carry items up to approx. 30 pounds.
