The Outreach Team is supported by CHN's compassionate health care team. Each of these health care professionals bring quality health care to people througout Northwest Indiana. The following services are provided in places of worship, Head Start centers, schools, shelters, public housing communities, apartment complexes, community centers, and at community events:

Mobile Unit Services
CHN's Mobile Medical Clinic is on a mission to provide essential medical, dental, and behavioral health services to both children and adults in Northwest Indiana. This initiative focuses on providing accessible care to those who have often been underserved in these vital areas of care.
