Community Health Worker - Behavioral Health (Full-time, 40 hours per week)

Come join our growing Healthcare Employee Team! Community HealthNet, Inc., a Federally Qualified Health Center Organization (FQHC), with 6 health centers in Lake County, Indiana, only 30 minutes from downtown Chicago, is looking to employ full-time Community Health Workers to join our integrated patient care team.

Key Responsibilities:

  • Perform proactive outreach to identify potentially eligible clients/patients
  • Responsible for proactive identification of clients and patients with depression, anxiety, diabetes, or other chronic health conditions to connect, coordinate and provide needed services and self-management support.
  • Engage clients/patients to help them move from a passive role to an active role in their health.
  • Utilize PRAPARE to address patients’ social determinates of health.
  • Function in a multidisciplinary team environment to implement evidence-based care for youth and adult primary care patients.
  • Proactively monitors population/caseload to meet treatment intensification guidelines.
  • Use effective engagement strategies to build a trusting relationship with patients and family members for ongoing partnership.
  • Facilitate patient access to information, including formal and informal support systems, web-based, and print information.
  • Make appropriate referrals to health and social service agencies. Referrals may include application assistance and client advocacy within the CHN organization and/or to external organizations.
  • Develop, manage, and facilitate support groups as needed.
  • Assist patients in obtaining medications through prescription assistance programs and other means.
  • Provide behavioral health screenings and perform vital signs assessments, as needed, under the supervision of a licensed medical provider (training will be provided).
  • Maintain appointments with patients/clients to provide them and their families with support and education.
  • Participate in training, continuing education and certification programs.
  • Function as a primary point of contact for some clients/patients.
  • Proficiently use and maintain an Electronic Health Record and/or registry for population management, including active follow-up for enrolled patients.
  • Systematically review the caseload with the care team each week, focusing on new patients/clients and/or patients/clients who have not adequately improved as expected.

Requirements:

  • Associates or Bachelor’s Degree in Health, Social or Human Services, Bachelor's preferred
  • Two years related experience preferred, or combination of education and experience;
  • Certification as a Community Health Worker or the ability and willingness to be trained and certified as a Community Health Worker Required
  • Strong communication skills, both written and verbal
  • Bi-lingual English/Spanish preferred
